How do we collect information from you?
We obtain information about you when you complete a patient questionnaire prior to your first appointment.Personal information to aid in understanding your particular state of health and well being will also be taken during your appointments.
What type of information is collected from you?
The personal information we are likely to collect might include your health concerns, health history and possible contributing factors to your state of health and well being. It might also include details of other health professionals e.g. homeopath, doctor etc. you have seen or are seeing.How this personal information is held securely is explained below.
We review our retention periods for personal information on a regular basis. We are legally required to hold some types of information to fulfil our statutory obligations. We will hold your personal information on our systems for as long as is necessary for the relevant activity, or as long as is set out in any relevant contract you hold with us.
Who has access to your information?
We will not sell or rent your information to third parties.
We will not share your information with third parties for marketing purposes.
HCHC practitioners working within the Homeopathic Community Health Clinics may have access to a patient's personal information for the purposes of providing health services. This access is restricted to information that is necessary to deliver the service and we have contracts in place that require each practitioner to keep your information secure and used only for the delivery of the homeopathic health care service agreed. Students working within the Homeopathic Community Health Clinics may have limited access to a patient's personal information. We have contracts in place that require each student to maintain the confidentiality of any and all personal information to which they may have access as part of the HCHC training.
Third Party Service Providers working on our behalf
We will not release your information to third parties, such as doctors or other health professionals unless you have requested us to do so, or we are required to do so by law, for example, by a court order or for the purposes of prevention of fraud or other crime.

Security precautions are in place to protect the loss, misuse or alteration of your information.
When you give us personal information, we take steps to ensure that it’s treated securely. All paper copies are kept under lock and key. By law, we are required to keep all patient records for seven years. If they are no longer current, they are destroyed securely.
Non-sensitive details (your email address etc.) are transmitted normally over the Internet, and this can never be guaranteed to be 100% secure. As a result, while we strive to protect your personal information, we cannot guarantee the security of any email information you transmit to us, and you do so at your own risk.
